Title: Modulation der Aktivierung von Thrombozyten und Monozyten durch Thrombin und Thrombospondin-1

Abstract: Die Serinprotease Thrombin spaltet Fibrinogen zu Fibrin, aktiviert Thrombozyten und die Gerinnungsfaktoren V, VIII, XI und XIII und stimuliert so die Thrombozyten-abhängige Thrombingenerierung. Mit Hilfe der kalibrierten automatisierten Thrombographie (CAT) wurde bestätigt, dass die positiven Rückkopplungsschleifen der Gerinnungsfaktoren IX und XI (FXI) essenziell für die Thrombin-induzierte Amplifizierungsphase der Thrombingenerierung auf Thrombozyten sind. Ebenso wurde mit Hilfe der CAT gezeigt, dass der klassische Thrombospondin-1-(TSP-1-)Rezeptor CD36 eine wichtige Rolle bei der Amplifizierung der FXIa-getriebenen Thrombingenerierung auf humanen Thrombozyten spielt. Dabei ist die CD36-sensitive Thrombingenerierung von FXI, Fibrin und GPIbα- sowie Syk-mediierten Signalwegen abhängig. CD36 stellt somit ein antithrombotisches „Target“ dar, um die Thrombozyten-basierte Hyperreaktivität zu hemmen. Neben Thrombozyten sind auch „Tissue Factor“-präsentierende Monozyten in der Lage, geringe Mengen an Thrombin zu generieren. Durch den Einsatz humaner Monozyten in der CAT wurde bestätigt, dass eine initiale Thrombingenerierung in Thrombozyten-freiem Plasma (PFP) dosisabhängig auf Monozyten erfolgt, für die Amplifizierung der Thrombingenerierung jedoch Thrombozyten essenziell sind. Zudem wurde bestätigt, dass Dabigatran die Thrombinamplifizierung auf Thrombin-stimulierten Thrombozyten verringert. Thrombin spielt ebenso eine wichtige Rolle bei der Atherogenese. Mit Hilfe eines etablierten Flusskammersystems wurde beobachtet, dass Dabigatran zu einer reduzierten Rekrutierung von Monozyten an eine Endothelzellschicht beiträgt. Es könnte daher bei der medikamentösen Behandlung von entzündlichen, atherosklerotischen Erkrankungen hilfreich sein. Es sind jedoch auch Thrombin-unabhängige Mechanismen an der endothelialen Rekrutierung und Transmigration von Monozyten beteiligt. Unter Verwendung des Flusskammersystems wurde festgestellt, dass die endotheliale Transmigration von murinen Makrophagen von dem matrizellulären TSP-1 abhängig ist. Die genauen Mechanismen der TSP-1-abhängigen Transmigration bleiben zu erforschen. Da TSP-1-präsentierende Monozyten/Makrophagen inflammatorische Erkrankungen prothrombotisch beeinflussen können, stellt TSP-1 ein potenzielles „Target“ bei der medikamentösen Behandlung von thrombotisch-inflammatorischen Erkrankungen wie der Atherosklerose dar.
The serine protease thrombin cleaves fibrinogen to fibrin, activates thrombocytes and coagulation factors V, VIII, XI and XIII and thus stimulates thrombocyte-dependent thrombin generation. Calibrated automated thrombography (CAT) confirmed that the positive feedback loops of coagulation factors IX and XI (FXI) are essential for the thrombin-induced amplification phase of thrombin generation on platelets. CAT was also used to show that the classic thrombospondin-1 (TSP-1) receptor CD36 plays an important role in the amplification of FXIa-driven thrombin generation on human platelets. The CD36-sensitive thrombin generation is dependent on FXI, fibrin and GPIbα- and Syk-mediated signaling pathways. CD36 thus represents an antithrombotic target to inhibit platelet-based hyperreactivity. In addition to thrombocytes, monocytes presenting “tissue factor” are also able to generate small amounts of thrombin. The use of human monocytes in CAT confirmed that an initial generation of thrombin in platelet-free plasma (PFP) occurs dose-dependently on monocytes, but that platelets are essential for the amplification of thrombin generation. In addition, it has been confirmed that dabigatran decreases thrombin amplification on thrombin-stimulated platelets. Thrombin also plays an important role in atherogenesis. Using an established flow chamber system, it was observed that dabigatran contributes to reduced recruitment of monocytes to an endothelial cell layer. It could therefore be helpful in the drug treatment of inflammatory atherosclerotic diseases. However, thrombin-independent mechanisms are also involved in endothelial recruitment and monocyte transmigration. Using the flow chamber system, it was found that endothelial transmigration of murine macrophages is dependent on the matricellular TSP-1. The exact mechanisms of TSP-1 dependent transmigration remain to be explored. Since TSP-1-presenting monocytes/macrophages can have a prothrombotic effect on inflammatory diseases, TSP-1 represents a potential target in the drug treatment of thrombotic-inflammatory diseases such as atherosclerosis.
