Abstract: | Einleitung und Literaturdiskussion: Patienten mit einer einseitigen Ertaubung (Single
Sided Deafness, SSD) oder einem asymmetrischen Hörverlust (Asymmetric
Sensorineural Hearing Loss, ASHL) haben im privaten, schulischen und beruflichen
Alltag große Einschränkungen im Vergleich zu Normalhörenden. Seit etwas mehr als
einer Dekade besteht für diese Patienten eine Therapieoption in der Versorgung mittels
Cochlea Implantat (CI). Ziel dieser retrospektiven Studie war es daher zu untersuchen,
ob die subjektive Ertaubungsdauer vor Implantation Einfluss auf die postoperativen
Hörerfolge von Patienten mit SSD/ASHL hat und ob diese von der Operation profitieren.
Material und Methoden: Es wurden insgesamt 50 Patienten identifiziert, die im
Zeitraum zwischen Dezember 2009 und Juni 2017 in der Hals-, Nasen-, Ohrenklinik und
Poliklinik der Universitätsmedizin der Johannes Gutenberg-Universität Mainz bei
einseitiger Schwerhörigkeit mit einem CI versorgt worden sind. In dieser Studie wurden
Patientendaten, audiologische Daten und Quality of Life (QoL)-Bögen erfasst. Eine
Gruppe von n = 37 Patienten wurde routinemäßig 3, 6 und 12 Monate postoperativ
audiometrisch untersucht und die Ergebnisse mit der subjektiven Ertaubungsdauer
korreliert. Ebenfalls erfolgte eine Befragung bezüglich der Lebensqualität der
implantierten Patienten. Die Auswertung erfolgte mithilfe einer Datenmaske in MS Excel,
Systat SigmaPlot und IBM SPSS Statistics.
Ergebnisse: Im untersuchten Kollektiv wurde auf dem zur CI-Implantation anstehenden
Ohr präoperativ ein Einsilberverständnis von 0,6 (±3,5) % bei 65 dB SPL im Freifeld bei
vertäubtem Gegenohr erreicht (Freiburger Sprachaudiometrie). Im Rahmen der
Erstanpassung nach 3 Monaten stieg dieses Ergebnis auf 27 (±31) % und verbesserte
sich nach 12 - 36 Monaten auf 42 (±26) %. Die subjektive Ertaubungsdauer betrug zum
Zeitpunkt der Implantation 158 (± 172) Monate. Es zeigte sich keine signifikante
Korrelation zwischen den sprachaudiometrischen Ergebnissen und der subjektiven
Ertaubungsdauer (R² = 0,284, rs (nach Spearman-Rho) = -0,348, p = 0,065). Im Bern
Benefit in Single Sided Deafness Questionnaire (BBSS) zeigte sich in allen abgefragten
Items ein positives Ergebnis für das Hören mit Hörhilfe mit einem Median >1,3 auf einer
Skala von -5,0 bis +5,0. Das Item zur Beurteilung des zusammengefassten Höreindrucks
zum Hören mit Hörhilfe ergab einen Median von 4,0.
Diskussion: Die CI-Versorgung ist ein sinnvoller Therapieansatz zur Hörrehabilitation
bei SSD/ASHL. Die subjektive Ertaubungsdauer vor Implantation allein stellt
wahrscheinlich keinen negativen prädikativen Faktor dar und ist somit generell nicht als
alleinige Kontraindikation gegen eine Versorgung zu werten. Objective: Patients who suffer from Single Sided Deafness (SSD) or Asymmetric Sensorineural Hearing Loss (ASHL) face great problems and social isolation concerning their private, educational and professional life. For a little more than a decade these patients have the option to get a cochlear implant (CI) to treat this condition. The aim of this retrospective study was to evaluate if the subjective time of hearing impairment/loss before implantation influences the post-surgical outcome of patients with SSD/ASHL and if they can profit from this treatment. Methods: Fifty patients underwent a CI implantation after being diagnosed with unilateral hearing loss between December 2009 and July 2017 at the University of Mainz, School of Medicine, Department of Otolaryngology, Head and Neck Surgery. This study recorded patient data, results of pure-tone audiomtery (PTA) and speech audiometry (Freiburger Sprachaudiometrie, Freiburger speech audiometry). A group of n = 37 patients were examined with standard audiometric tests 3, 6 and 12 months after the surgery. Also, we gathered information via admitting the Bern Benefit in Single Sided Deafness Questionnaire (BBSS) as self-assessment tool for the Quality of Life (QoL) of implanted patients to the examined collective. MS Excel, Systat SigmaPlot and IBM SPSS Statistics were used for the evaluation of the data. Results: The examined group showed an understanding of one-syllable words with the pre-implanted ear in the Freiburger speech audiometry of 0.6 (±3.5) % at 65 decibel (dB) sound pressure level (SPL) in quiet with a masked contralateral ear. After 3 months, the phase of first adjustments, this result got to 27 (±31) % and improved to 42 (±26) % after 12 - 36 months after the surgery. The subjective time of hearing loss at the time of implantation was 158 (± 172) months. No evidence for a significant correlation between the results of the speech audiometry and the subjective time of hearing loss could be detected (R² = 0,284, rs (Spearman-Rho) = -0,348, p = 0,065). The BBSS showed positive results in all items presented to the patient with medians above 1.3 on a scale from -5.0 to 5.0. Representing the general hearing impression with a hearing aid item number 10 scored a median of 4.0. Conclusion: The use of a CI as treatment for patients with SSD/ASHL is a reasonable option. The subjective time of hearing loss before CI implantation alone probably isn’t a negative predicative factor and shouldn’t be used as general contra indication against CI-surgery for patients with SSD/ASHL. |
