Abstract: Es wurde eine retrograde Analyse von Patientenakten der Schmerzambulanzder Klinik für Anästhesiologie der Universitätsklinik Mainz durchgeführt, indie alle Patienten mit bestimmten Einschlußkriterien derBehandlungsjahrgänge1996 und 1997 aufgenommen wurden.Dies waren die vier Diagnosegruppen multilokuläre Schmerzen,Rückenschmerzen, Phantomschmerz und Morbus Sudeck (SRD). Das Ziel dervorliegenden Arbeit war die Frage nach der Häufigkeit von Psychotherapie alsergänzende Therapieempfehlung seitens der Schmerzambulanz herauszuarbeiten.Psychotherapie (ambulant, stationär, Bestandteil vonRehabilitationsaufenthalten) in vielgestaltiger Weise wurde häufigerempfohlen, 1. je länger die Schmerzerkrankung bestand,2. je jünger die Patienten waren,3. je länger sie arbeitsunfähig waren,4. wenn belastende biographische Ereignisse festgestellt werden konnten5. je höher das Chronifizierungsstadium nach Gerbershagen war. Im Einzelnenspielten die zeitlichen Aspekte der Erkrankung, Lokalisationseinflüsse sowieAspekte vorheriger Behandlungen und schmerzbedingter Krankenhausaufenthalteeine besondere Rolle.6. wenn Patienten nicht berentet waren.
A retrograde analysis of patient files from the out-patient pain departmentof the clinic for anaesthesiology of the university clinic in Mainz wascarried out, whereby all patients from the treatment years 1996 and 1997 whofulfilled certain criteria were included in this study.These were the four diagnostic groups of multilocular pain, dorsalgia,phantom pain and Sudeck's disease (sympathetic reflex dystrophy SRD). Thegoal of this study was to investigate the frequency of psychotherapy asrecommended supplementary treatment on the part of the out-patient paindepartment. Psychotherapy (on an in- or out-patient basis as well as part ofrehabilitation courses) was frequently recommended in diversified forms:1. the longer the state of pain persisted;2. the younger the patients were;3. the longer the patients were unable to work;4. when burdening biographic incidents were found;5. the greater the chronification status according to Gerbershagen, wherebyin detail the temporal aspects of the disorder, localising influences aswell as aspects of previous therapy and pain-conditioned periods ofhospitalisation played a special role;6. if the patients were not pensioned.
