Title: | Charakterisierung parenteraler Eisenkomplexe: Untersuchungen zu Physikochemie, In-vitro-Verhalten und Austauschbarkeit von Eisen „non-biological complex drugs“ |

Abstract: | Trotz ihres jahrzehntelangen Einsatzes in der klinischen Therapie ist die Struktur von parenteralen Eisenkomplexen noch nicht abschließend erforscht. Auf Grund ihres komplexen Aufbaus und Herstellprozesses werden sie zu den non-biological complex drugs gezählt. Im Rahmen dieser Arbeit wurden sechs unterschiedliche parenterale Eisenkomplexe und ein Entwicklungsprodukt in Hinblick auf Struktur, Stabilität, Patientensicherheit und Austauschbarkeit untersucht. Des Weiteren wurde ein direkter Vergleich von Eisengluconat (Originator) zu seinem auf dem nordamerikanischen Markt befindlichem Nachfolgeprodukt durchgeführt. Die Präparate wurden durch ihre physikochemischen Eigenschaften, ihre thermodynamische Stabilität und ihr Freisetzungsverhalten charakterisiert. Bei vier Produkten wurden in einem in-Ovo-Modell Biodistributionsstudien der pharmakologisch und toxikologisch relevanten Organe Leber und Herz durchgeführt. Es zeigten sich klare Unterschiede zwischen den untersuchten Komplexen. Präparate mit Polyol-Ligand (Dextran, Derisomaltose und Polyglucose-Sorbitol-Carboxymethylether) zeigten insgesamt eine höhere Stabilität als solche mit Di- oder Monosaccharide-Ligand (Sucrose, Glukose). Ein linearer Zusammenhang zwischen Komplexstabilität und der Eisenherzkonzentration im In-ovo-Modell konnte festgestellt werden. Insgesamt legen die Ergebnisse dieser Arbeit nahe, dass keine direkte Austauschbarkeit zwischen parenteralen Eisenpräparate gegeben ist. Although parenteral iron drugs have been introduced to clinic decades before, their structure isn’t fully characterized yet. Due to their complex structure and demanding manufacturing process, they are considered non-biological complex drugs. Six commercially available parenteral products and a development product have been investigated in terms of structure, stability, patient safety and exchangeability. Moreover, a direct comparison of iron gluconate and its North American follow-up version has been conducted. Products were characterized in their physicochemical properties, thermodynamic stability and in-vitro iron release. For four products, iron distribution in the pharmacological and toxicological relevant tissues liver and heart was tested in a chicken fetus model. Clear differences were found between the investigated products. Complexes with polyol ligand (dextran, isomaltoside, polyglucose sorbitol carboxymethylether/oxytol) showed higher stability than those with di- or monosaccharide ligands. A linear connection between complex stability and iron heart concentration in the chicken model was observed. Summarized, the results from this work indicate that there is no direct exchangeability between different iron products. |
