dc.description.abstractHintergrund: Intensivpatienten erleiden oftmals zeitgleich Infektionen und ein akutes Nierenversagen, das mit kontinuierlicher venovenöser Hämodialyse (CVVHD) oder Hämodiafiltration (CVVHDF) behandelt wird. Für diesen Fall fehlen gültige Dosierungsangaben für Antibiotika. Es besteht das Risiko einer inadäquaten Therapie mit Unwirksamkeit, unerwünschten Wirkungen oder Resistenzentwicklung. Ziel dieser klinischen Studie war, die Plasmaspiegelverläufe resultierend aus der Standardtherapie von Linezolid und Meropenem bei Intensivpatienten mit CVVHD(F) zu erfassen und zu bewerten. Methoden: Es wurden chirurgische Intensivpatienten eingeschlossen, die eine CVVHD(F) und zugleich Linezolid (2 x 600 mg tgl.) und/oder Meropenem (3 x 1g tgl.) erhielten. Während eines Dosierungsintervalls im Steady-State wurde zu festgelegten Zeitpunkten Blut und Dialysefiltrat entnommen. Die Quantifizierung erfolgte per HPLC-UV. Anhand der errechneten pharmakokinetischen Parameter wurde die Therapie bewertet und die theoretisch optimale Tagesdosis kalkuliert. Ergebnisse: Es wurden pro Antibiotikum 20 Plasmaspiegelverläufe bestimmt. Linezolid zeigte interindividuell schwankende Plasmaspiegel, im Mittel vergleichbar mit den Angaben in der Fachinformation, jedoch bei 80% der Patienten unterdosiert. Meropenem war bei allen Patienten adäquat dosiert. Schlussfolgerungen: Linezolid ist bei Intensivpatienten mit CVVHD(F) unterdosiert. Die Dosis sollte erhöht und die Therapie durch TDM überwacht werden. Die Regeldosierung von Meropenem ist auch bei begleitender CVVHD(F) ausreichend.de_DE
dc.description.abstractBackground: Intensive care unit (ICU) patients often suffer from infections and acute renal failure and might need continuous venovenous hemodialysis (CVVHD) or hemodiafiltration (CVVHDF). There are no dosage recommendations for antibiotics in this case. Risk for therapy failure, development of resistance and adverse drug effects is elevated. Aim of this study was to find out if standard therapy of Linezolid and Meropenem is adequate in surgical ICU patients receiving CVVHD(F). Methods: Surgical ICU patients receiving CVVHD(F) and 600 mg Linezolid b.i.d. and/or 1 g Meropenem t.i.d. were enrolled in the study. We determined steady state plasma and ultrafiltrate concentrations throughout one dosing interval using HPLC-UV. Optimal dosing regimens were calculated from the derived pharmacokinetic data. Results: 20 patients were recruited for each antibiotic. Linezolid showed interindividual variability of plasma levels with 80% of the patients rated as underdosed although mean plasma levels were comparable to the data in the product information. Meropenem showed adequate plasma levels and extended elimination half-life. Conclusions: Linezolid is underdosed in ICU-patients receiving CVVHD(F). Dose adjustment to 600 mg t.i.d. and therapeutic drug monitoring might be useful. Meropenem standard dose is appropriate in CVVHD(F)-patients.en_GB
